Evergreen Fountains Assisted Living wing is a two-story building attached to the main building. The apartments are luxurious and spacious with a variety of sizes, featuring studio, one-bedroom, and two-bedroom floor plans.
Each resident receives an individual assessment and a customized care plan. Evergreen Fountains works along with your personal physician to meet your medication needs. We offer the assistance our residents need to aid them in maintaining a healthy lifestyle. Our Evergreen Fountains Nurse Director determines the final assessment and which program would benefit the individual resident’s needs.
